Immunizations

Immunization Requirements – Fall 2013 and Spring 2014

To ensure the health and safety of our campus, immunization against communicable diseases is extremely important. The following vaccinations are required of all first-time freshmen, and all transfer students:

  • MMR (Measles, Mumps, Rubella) (Two doses)
  • Meningococcal Vaccine
  • Polio series
  • Tetanus-Diptheria-Pertussis (DTaP, Tdap, DT and Td)
  • Hepatitis B series

Additionally, the following immunizations are recommended prior to enrollment at WVU:

  • Varicella vaccine (chicken pox)
  • Tuberculosis screening
  • HEPATITIS A vaccine or combined HEPATITS A&B vaccine
  • Annual Influenza vaccines
  • Pneumovax, if indicated
  • HPV series – Gardasil (for females and males)
  • Most students enrolled in a health care major will be required to have the Pertussis vaccination

Documentation of Immunizations

Documentation of these immunizations must be submitted to WELLWVU Student Health prior to enrollment in your initial entry semester.

2013-2014 Academic Year

Please complete the 2013-2014 Immunization Form and return it to:

2014 – 2015 Academic Year

If you are enrolling for the Fall 2014 or Spring 2015 semesters, you must submit documentation prior to enrollment in the Fall 2014 or Spring 2015 semester.

Acquiring Immunization Records

Payment

Credit or debit cards, cash or check, Student Account Charge, Mounty Bounty, are payment options. At this time, Student Health does not bill insurance companies. You may keep your receipt and forward it to your insurance company for possible reimbursement.

Summer and Semester Breaks Monday – Friday 8:30AM – 4:30PM
