Immunization Requirements

Fall 2011 and Spring 2012

To ensure the health and safety of our campus, immunization against communicable diseases is extremely important. Vaccinations against Measles, Mumps and Rubella (MMR) and Meningococcal are required of all transfer students. In addition to the MMR, the following vaccinations are required of all first-time freshmen:

Documentation of these immunizations must be submitted to the Office of Admissions prior to enrollment in the Fall 2011 or Spring 2012 semester, whichever is your initial entry semester.

Please complete the 2011-2012 Immunization Form and return it to:

Acquiring Immunization Records

Payment

Student Account Charge, Mountie Bounty, are payment options. At this time, Student Health does not bill insurance companies. You may keep your receipt and forward it to your insurance company for possible reimbursement.
