• If you need a copy or a specified portion of your medical record, this legal document allows Student Health to provide it to you or the health care facility of your choice. An original signature of the student is required:
  AUTHORIZATION FOR RELEASE OF INFORMATION-STUDENT HEALTH
  AUTHORIZATION OF RELEASE OF INFORMATION-PSYCHIATRY

• Prior to matriculation, vaccination against Measles, Mumps, Rubella (MMR), Meningitis, Polio, Tetanus, and Hepatitis B is required of all first time freshmen, and all transfer students. Documentation of these must be provided to WELLWVU Student Health by submitting WVU’s Immunization Form.

• Must be submitted by student or allergist to receive allergy injections at Student Health. ALLERGY CONSENT FORM
